Here you create the questions the recipients of the survey shall answer. And you define how these answers shall look like: Shall the recipient write a text, tick checkboxes with pre-defined answers or select only one pre-defined answers? You can also state that certain questions must be answered (mandatory) and you can request that the recipient adds some documentation.

Question type:Select dependent on the type of answer you expect.
Text
The recipient shall answer with a short free text. One line.
Text area
A longer free text area where the user can write several lines of text.
Date
The recipient answers by picking a date from a calender.
Checkbox
The recipient can select several answers. Enter possible answers via the list below: Enter the first answer under Title and then add as many possible answers as necessary by clicking Add answer.
Radio button
The recipient can select one answer. Enter possible answers via the list below: Enter the first answer under Title and then add as many possible answers as necessary by clicking Add answer.
Number
The recipient answers by adding a number. In case they enter a letter, they will get an error message.
Dropdown
The recipient can select the correct answer from a dropdown list. Enter possible answers via the list below: Enter the first answer under Title and then add as many possible answers as necessary by clicking Add answer.
Text field for complimentary description: If Yes is selcted, an additional text, for example a tip, will be displayed to explain the question.
Attach file to the response? Here you can define that the recipient can or must add documentation to their answer.
